Skilled Hospice Nursing — African Nurse Caring for Senior Patient

Skilled Hospice Nursing (RN/LPN) — 24/7 On-Call

Our licensed Registered Nurses and Licensed Practical Nurses form the backbone of every patient's care plan. They conduct regular home visits to monitor the patient's condition, administer medications, manage medical equipment, and educate families on what to expect at each stage of the hospice journey.

Hospice nursing goes far beyond clinical tasks. Our nurses build trusting relationships with patients and families, serving as a steady source of knowledge, comfort, and reassurance. They are trained to recognize subtle changes in a patient's condition, respond proactively, and communicate clearly with the rest of the care team so that nothing falls through the cracks.

Because needs don't follow a schedule, our nursing team is available around the clock — 24 hours a day, 7 days a week — so patients and families always have a skilled professional just a phone call away, including nights, weekends, and holidays. Whether it is a question about a medication, a sudden change in symptoms, or simply the need for guidance at three in the morning, we are here.

Frequently Asked Questions

How often will a nurse visit my loved one?

Visit frequency depends on the patient's condition and care plan. Most patients receive nursing visits several times per week, but visits can increase to daily or even more frequently if symptoms change or a crisis develops. Your care team will discuss the schedule with you and adjust it as needs evolve.

What is the difference between an RN and an LPN in hospice?

Both Registered Nurses and Licensed Practical Nurses are trained, licensed professionals. RNs handle the initial patient assessments, develop and update care plans, manage complex medications, and coordinate with the physician. LPNs provide ongoing skilled nursing care, administer medications, monitor symptoms, and report changes to the RN. Together, they ensure comprehensive, continuous coverage.

What happens if I call the on-call line in the middle of the night?

You will speak directly with a licensed hospice nurse — not a voicemail or answering service. The nurse will assess the situation over the phone and, if needed, can dispatch a nurse to the home at any hour. No concern is too small, and families are always encouraged to call rather than wait.

Will the same nurse visit each time?

We do our best to assign a consistent primary nurse to each patient so that a trusting relationship can develop. While scheduling and on-call rotations may occasionally bring a different team member, every nurse on our staff has access to the patient's full care plan and is prepared to provide seamless, informed care.

Does the nurse bring medications and supplies?

Our nurses coordinate all hospice-related medications, medical equipment, and supplies as part of the care plan. Medications are typically delivered to the home by a hospice-contracted pharmacy, and the nurse ensures the family understands how and when to administer them. If supplies run low or a new need arises, the nurse can arrange delivery promptly.
