Who We Serve
Good Living Care Services Hospice serves patients who have been diagnosed with a life-limiting illness and have chosen to focus on comfort rather than curative treatment. Our patients include individuals living with advanced cancer, heart failure, chronic obstructive pulmonary disease (COPD), Alzheimer's disease and other dementias, kidney disease, liver disease, ALS, stroke-related conditions, and other serious illnesses.
We serve patients wherever they call home — private residences, assisted living communities, memory care facilities, and skilled nursing facilities throughout Brooklyn Park and the surrounding Minnesota region. Our team comes to the patient; the patient never has to come to us.
We also welcome referrals from physicians, hospitals, discharge planners, case managers, and families themselves. You do not need a doctor's referral to call us and ask questions — we are happy to help you understand your options and, if appropriate, coordinate with your physician to begin the process.

What Is Hospice?
Hospice is a specialized form of medical care designed for individuals who are living with a terminal illness and have chosen to focus on comfort, quality of life, and dignity rather than curative treatments. It means redirecting the full resources of a professional care team toward keeping the patient comfortable and supporting the family.
Who Is Eligible?
A patient may be eligible for hospice care when a physician certifies that, if the illness follows its expected course, the patient's life expectancy is six months or less. Eligibility does not mean the patient has only six months to live — many patients receive hospice care for longer — and patients may revoke hospice at any time.
When Should You Call?
Many families wait too long to explore hospice because they are unsure whether the time is right. It may be time to consider hospice if the patient has been hospitalized multiple times recently, condition is declining despite treatment, daily activities have become significantly more difficult, or the family is feeling overwhelmed.

If a patient's condition stabilizes or improves, they can be discharged from hospice and return to curative treatment at any time. Hospice can always be resumed later if needed. There is no penalty for leaving and no barrier to coming back.

For most patients, there is no out-of-pocket cost. The Medicare Hospice Benefit covers all hospice-related services, medications, equipment, and supplies. Medicaid and most private insurance plans provide similar coverage. We verify benefits and handle all paperwork on the family's behalf.
Our bereavement team provides grief support to the family for up to 13 months, including counseling, support groups, educational resources, and regular check-ins. The family is never simply "discharged" — we remain a source of care and support for as long as needed.
