Our Story & Mission
GLC Hospice (Good Living Care) was founded on the belief that where care meets comfort, families find peace. The end of life is not a medical event to be managed — it is a sacred chapter that deserves the utmost dignity, compassion, and respect.

Our Story
Founded by Damaris Nyaben, GLC Hospice grew out of a firsthand experience with the gaps in end-of-life care. After witnessing a family member's struggle to find compassionate, reliable support during a terminal illness, our founder set out to build something better — a hospice that treats every patient like their own family.
Since our founding, we have had the privilege of serving many families across Hennepin County and the surrounding counties. Every family we serve strengthens our commitment to doing this work with excellence, integrity, and heart.
Our Mission
To provide compassionate, high-quality hospice care that honors the dignity of every patient, supports the well-being of every family, and ensures that no one faces the end of life alone or in unnecessary pain.
Our Vision
To be the most trusted hospice provider in Hennepin County and the surrounding counties — known for clinical excellence, genuine compassion, and an unwavering commitment to the patients and families we serve.
Our Core Values
Compassion
We lead with empathy in everything we do. We recognize that behind every diagnosis is a person with a lifetime of memories, relationships, and meaning, and we care for the whole person — not just the illness.
Dignity
Every patient has the right to be treated with respect, to have their wishes honored, and to live their final days on their own terms. We uphold that right without exception.
Integrity
We do what we say we will do. We communicate openly, act ethically, and hold ourselves accountable to the highest standards of hospice care. Families trust us with the most vulnerable moments of their lives, and we take that trust seriously.
Excellence
We pursue the highest standard in every clinical decision, every home visit, and every interaction. Our team members are carefully selected, rigorously trained, and committed to continuous improvement.
Family
We believe that caring for the patient means caring for the people who love them. Our support extends to the entire family unit — before, during, and after the loss of a loved one.
Accessibility
Quality end-of-life care should never depend on a family's financial situation. We are committed to removing barriers and ensuring that every eligible patient can receive the care they deserve.
Cultural Inclusivity & Multi-Lingual Respect
We honor every background, religion, and cultural tradition across Hennepin County — providing bilingual staff, certified interpreters in Somali, Spanish, and Hmong, and tailoring bedside care to your family's personal values and customs.

Our Team
GLC Hospice is powered by a team of dedicated professionals who have chosen this work because it matters to them personally — not just professionally. Our staff includes:
Registered Nurses and Licensed Practical Nurses with specialized training in hospice and palliative care, skilled in pain management, symptom control, and compassionate patient education.
Certified Home Health Aides who provide hands-on personal care with gentleness, patience, and respect for each patient's preferences and routines.
Licensed Medical Social Workers trained in grief counseling, family dynamics, advance care planning, benefits coordination, and community resource navigation.
Chaplains and Spiritual Care Providers who serve patients and families of all faiths — and those of no faith — with sensitivity, openness, and deep respect for individual beliefs.
Bereavement Counselors who support families through the grief process with individual counseling, support groups, and ongoing check-ins for up to 13 months after a loss.
A Hospice Medical Director who provides physician-level oversight of every plan of care, collaborating with each patient's personal doctor to ensure that clinical decisions are sound, ethical, and aligned with the patient's goals.
Trained Volunteers who offer companionship, practical assistance, and a reassuring presence to patients and families throughout the hospice journey.
Every member of our team undergoes thorough background checks, ongoing education, and regular performance reviews. We invest in our people because the quality of our care depends on the quality of the individuals delivering it.
We are fully licensed and certified to provide hospice care in the state of Minnesota, ensuring the highest standards of safety and compliance.
